Applications - Slide Request Form
(Beta testing NEW version - To be released in July of 2008)

We have discontinued the distributable version of our slide request form and will be replacing it with an online version with more features shortly. We feel it will allow users more options and be easier to use. For those of you still using the old version or requiring support, we will still provide all of the help you need for as long as you need it.

Some practical scenarios for its use will be:

• Standard requests for tissue analysis in a hospital for patient care
• Special requests for in-depth case review
• Research studies

It is still based on the previous version but will utilize your web browser rather than Adobe Acrobat. Any features that you have been accustomed to will still be available.

If you have not seen the slide request form, the previous version has been posted as an illustration; it will be replaced soon with an online eForm called the HistoSoft Slide Request eForm.
